Entry fee of $50.00 covers participation in Padded Contact Tournament and entry to the Full Contact Tournament.  Under no circumstances will the entry fee be refunded

Applicant's parent or guardian permission and signature must provided to the AKKA, either accompanying this application or to be received by last post Fri 05 Aug 2016 at AKKA, 130 Hewlett Street, Bronte, NSW 2024, Australia.

By entering the tournament as a contestant, the applicant is agreeing to and accepting the following:

1. In participating in the "2016 Australian Open Kyokushin Karate Championships" (the tournament), I am aware of and understand the risk of harm (including personal injury or death, damage to property or economic loss).
2. I am aware that this sport is a "dangerous recreational activity" that involves a significant risk of physical harm.
3. I agree to comply with all directions which may be given by the Australian Kyokushin Karate Association, Inc (or its' agents, employees or contractors) from time to time, with respect to my participation in the tournament.
4. I release and waive all liability in respect of the tournament (or its agents, employees or contractors), that may otherwise have arisen as a result of my participation in the tournament.
5. I understand that the tournament (or its agents, employees or contractors) will not be liable for the cost of treatment of any injuries that may be sustained as a result of participating in the tournament.
6. I agree to become a member of the "Australian Kyokushin Karate Association" (AKKA) for the duration of the tournament, the cost of which is included in the entry fee.
7. A doctor's certificate stating that the application is fit to participate in a full contact karate tournament, and that the applicant does not have any person to person transferable infections or diseases, must accompany the application.
8. Any photograph or video taken during this competition in which I appear is the property of the AKKA and I consent to the use by the organizer of any video or photograph as the AKKA deem appropriate and I waive any entitlement to any payment or compensation as a result of the use of any photograph or video by the AKKA in which I appear.
